About the role
- The position is for a lead physician to manage health care for 30 of our more complex and high-risk patients.
- This position will act as lead physician for a new "Intensive Case Management Program" and will include a separate clinic, collaborating closely with the other free clinics currently at GRACE.
- We anticipate operating 4 hour in-person clinic one afternoon per week.
- The lead physician will have administrative and nursing support in the clinic, and oversee continuing care provided by a full time nurse or paramedic working in the field to follow up, monitor and assist patients with their care plan.
Requirements
- Minimum – Active Florida Medical License
- be comfortable working in a low-resource setting, providing trauma-informed care for individuals facing significant medical and socioeconomic challenges.
- Preferred – 2-3 years experience working with at risk population.
