About the role

  • Support the DOH-Flagler Office of Community Health programs through an integrated and strategic approach to community outreach and projects directed toward prevention, assessment, risk reduction, and health status improvements for individuals, families, and the community.
  • Act as a support facilitator and deliver evidence-based Diabetes Prevention Program curriculum to multiple group cohorts, while promoting program participation through targeted outreach and engagement strategies.
  • Support a wide range of outreach activities designed to promote public health awareness and improve access to care, working closely with community partners to implement health education initiatives and DOH-Flagler services.
  • Participate in community outreach events, partner meetings, and health fairs to advance program goals, as well as assisting in the planning and coordination of health-related staff trainings.
  • Participate in general staff meetings, staff development activities, and continuing education to support ongoing professional growth and departmental effectiveness.
  • Perform related duties as needed, including assisting with public health services during disaster response efforts.
  • Ensure the timely submission of EARS and timesheets to maintain compliance with administrative requirements.

Requirements

  • Must be willing to accept this salary if offered the position

  • Must have current authorization to work in the United States without employer sponsorship
  • Experienced with Microsoft Office
