About the role
- Coordinates the plan of care to achieve desired outcomes and integrates the work of the health care team
- Ensures the development of a safe and effective plan of care through early identification and assessment of the patient's needs
- Collaborates by working with patients, families, caregivers, and the health care team locally and within the SHC system
- Arranges services among community agencies, physicians, patients, family, caregivers, and others involved in the plan of care
- Ensures prudent utilization of all resources by evaluation of the resources available to the patient
- Documents assessments, interventions, plans and evaluations in accordance with regulatory agency requirements and SHC policy
- Other duties as assigned.
Requirements
- BSN required, MSN preferred
- RN license in state of practice required
- Case Management Certification (CMC, ACM or ANCC) is required, or willingness to achieve within 12 months of eligibility
- 3 years Nursing experience required
- 5+ years of clinical experience in Pediatrics, Burn care and/or Orthopaedics preferred
- 1 year Care Coordination/Case Management experience is preferred
- Experience with utilization review/management with a working knowledge of InterQual criteria is strongly preferred.
