• The Product Designer owns user experience and user interface design tasks and projects, from requirements definition to go-live, for Sharp.com, the Sharp mobile app and other digital initiatives.
• Position involves building working partnerships across the business and within the Digital Experience, Creative Services and Web Development teams to assist creating and launching initiatives/projects on scope and on schedule.
• Creates sitemaps, design system assets, navigation schemes, wireframes and prototypes, to produce the best possible products for our patient and consumer population.

• Responsible for leading and optimizing the financial performance of Sharp Rees-Stealy's retail businesses, programs, and centers.
• Focus on strategic financial planning, operational efficiency, and maximization of profitability of these service areas.
• Has additional responsibilities associated with capital tracking, acquisition, and maintenance of incentive-based report cards.

Case Manager – Population Health, RN

Human Resources

• Provide management of patients transitioning from the inpatient care setting to home.
• Involves patient assessment and coordination of services in the immediate post discharge phase.
• Addresses care coordination, facilitation of referrals to providers, vendors, and community services, disease education, caregiver support and self-care counseling.
• Responsible for evaluation and if necessary modification of patient care plan based on post discharge assessment addressing medical/psychosocial status changes and facilitating the patient/caregiver/family goals to align with patient's new identified problems/barriers and needs.
• Results in decrease in avoidable readmissions and emergency department visits as well as increased patient and provider satisfaction.
