Intake Coordinator

• Handle general inquiry calls and emails from potential participants and referral sources
• Gather information from potential participants to complete detailed intake forms
• Submit Medical Treatment Plans to participants’ medical physicians and follow up
• Conduct in-person and virtual presentation outreach in the community
• Assist participants in applying for and renewing San Francisco Paratransit Group Van Transportation Service
• Assist with and maintain participants’ CACFP eligibility annually
• Maintain and input daily meal records
• Maintain participants’ Adult Day Services Participant Agreements
• Provide administrative support to the Director, including preparing statistical reports and performing office administration
• Perform other duties as assigned
