About the role
- Help clients gain access to needed medical, social, educational, and other services
- Conduct assessments of eligible clients to determine service needs
- Develop individualized integrated care plans
- Coordinate all services for each individual client, including advocacy and information about community resources
- Document services, including identifying resources, writing progress notes, and submitting as required
Requirements
- BA degree in human service field, preferably Social Work
- Case Management certification in proper jurisdiction required
- 2 years experience in providing services to victims of crime including trafficking and sexual assault
- Must have a valid state driver’s license
