Provides direct services to meet the psychosocial needs of patients and their families, including middle and older adults at SACC
Provides support to the patient care areas
Deliver psychosocial assessments, care coordination, and discharge planning support as needed
Collaborate with clinical teams to support patient care and transitions
Requirements
Bachelor's Degree
One (1) year recent experience
Preferred: Discharge planning
