About the role
- Provides direct services to meet the psychosocial needs of patients and their families, including middle and older adults at SACC
- Provides support to the patient care areas
- Deliver psychosocial assessments, care coordination, and discharge planning support as needed
- Collaborate with clinical teams to support patient care and transitions
Requirements
- Bachelor's Degree
- One (1) year recent experience
- Preferred: Discharge planning
