About the role
- Perform primary functions of assessment, planning, facilitation and advocacy through collaboration with the client and other health care professionals involved in the client's care.
- Provide care coordination, evaluation, and advocacy for options and services to meet individuals' and families' comprehensive health needs.
- Communicate and utilize available resources to promote quality, cost-effective outcomes.
- Work within Providence Little Company Of Mary Torrance Hospital delivering patient-focused, whole-person care.
- Collaborate with multidisciplinary teams and participate in care management processes per CMSA guidelines.
Requirements
- Bachelor's Degree (All Registered Nurses without a minimum of a Bachelor’s degree in Nursing must obtain a Bachelor’s degree in Nursing or higher within 3 years of hire; exceptions considered case-by-case).
- Upon hire: California Registered Nurse License.
- 2 years Acute hospital experience, including 1 year of supervisory experience.
- Strong knowledge base or experience in specialty area assigned.
- Preferred: Master's Degree in Health care related field.
