Perform primary functions of assessment, planning, facilitation and advocacy through collaboration with the client and other health care professionals involved in the client's care.
Provide care coordination, evaluation, and advocacy for options and services to meet individuals' and families' comprehensive health needs.
Communicate and utilize available resources to promote quality, cost-effective outcomes.
Work within Providence Little Company Of Mary Torrance Hospital delivering patient-focused, whole-person care.
Collaborate with multidisciplinary teams and participate in care management processes per CMSA guidelines.
Requirements
Bachelor's Degree (All Registered Nurses without a minimum of a Bachelor’s degree in Nursing must obtain a Bachelor’s degree in Nursing or higher within 3 years of hire; exceptions considered case-by-case).
Upon hire: California Registered Nurse License.
2 years Acute hospital experience, including 1 year of supervisory experience.
Strong knowledge base or experience in specialty area assigned.
Preferred: Master's Degree in Health care related field.
