About the role
- Proactively engage with external stakeholders/strategic partner groups to gather feedback, identify process improvement opportunities and effectively deliver the CoCM to all of their patients.\n
- Demonstrate in depth knowledge of Concert Health and all of the companies offerings to provide both accurate information and solutions functionally across internal departments and external customers.\n
- Provide comprehensive administrative and clinical oversight to Collaborative Care Clinicians and Clinical Excellence Managers.\n
- Ensure diligent supervision of Collaborative Care Clinicians and other staff members within assigned care pods.\n
- Deliver ongoing education to care pods, ensuring adherence to the evidence-based Collaborative Care model and its core principles.\n
- Articulate clearly the fundamental principles and treatment options of the Collaborative Care model.\n
- Ensure that care pods meet or surpass key performance metrics related to clinical outcomes, quality patient care, and clinical efficiency.\n
- Collaborate effectively as part of a national team comprising sales, implementations, product development, quality assurance, and training.
Requirements
- Masters degree in social work, counseling, or related field.\n
- Independently licensed with the ability to successfully complete cross-licensing to CA and WA.\n
- Minimum 5+ years leadership, proven track record of meeting or exceeding clinical and operational KPISenior leadership experience with a proven track record of leading multi-state remote clinical teams.\n
- Proficiency in utilizing behavioral health tools such as PHQ9, GAD7, VARS, and related outcome measures.\n
- Skilled in Motivational Interviewing, Problem Solving Treatment, and Behavioral Activation.\n
- Documented proficiency with medical electronic health records like EPIC, Athena, ECW, etc.\n
- Experience in population health strategies, registry management, HEDIS measures, and value-based payment arrangements.\n
- Direct experience collaborating with primary care providers in primary care or related settings, with familiarity in chronic illness management and integrated care models in a leadership capacity.\n
- Exceptional organizational skills with a proven ability to manage clinicians across various disciplines.\n
- Demonstrated experience in "treat to target" models, specifically in Collaborative Care.\n
- Comfortable using computers and learning new technologies (EMRs, G-suite, care management platforms) while also adept at teaching others.\n
