About the role
- Communicate and collaborate closely with physicians and other members of the care team to provide consistent quality care for patients
- Accept responsibility and accountability for assigned patients as a clinic nurse and care manager to plan, implement, and evaluate the care for patients
- Assist patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes
- Work with a high degree of autonomy within a collaborative care team
- Develop all aspects of clinical workflow and care management operations within the medical home model
Requirements
- Graduate of an accredited school of nursing
- Associate’s Degree in Nursing
- Montana Registered Nurse License upon hire
- 3 years of Professional Nursing Experience
- 1 year of Care management or similar experience
- Preferred: Bachelor’s Degree in Nursing
