About the role
- Provide professional, comprehensive, patient centric care management services for at-risk patients in an acute care environment
- Conduct assessment and planning for inpatient care management
- Coordinate care across the continuum and with clinical teams
- Perform resource utilization management and/or review (utilization review)
- Lead discharge planning and ensure appropriate transitions of care
- Document interventions and ensure regulatory compliance
- Act as patient advocate
- Support goal of ensuring appropriate healthcare resources so care is delivered in the right setting at the right time
Requirements
- Associate's Degree - Nursing degree/diploma upon hire
- Texas Registered Nurse License upon hire - if practicing within this state
- 2 years of Acute care nursing experience in a Medical Surgical or Inpatient setting
- IRR or annual competency testing in Utilization Review
- Must be available to work weekends and holidays as scheduled
- Candidates must reside within commutable distance of the following office location: Texas: Lubbock
- This is an onsite position, requiring daily attendance at the designated location
- Preferred: Bachelor’s Degree in Nursing within 3 years of hire
- Preferred: 1 year of experience in care management or utilization review in any setting or successful completion of TIPS program or Case Management Orientation Program
