Assess needs and provide Certified Occupational Therapy Assistant (COTA) services and direct treatment in consultation with the Occupational Therapist.
Function as a member of the Interdisciplinary Team.
Responsible for home care coordination, including clinic work and home visits.
Collaborate with team to empower elders to live active, independent lives in the community.
Document care, coordinate with providers, and support restorative and longitudinal care goals.
Requirements
Graduate of a Certified Occupational Therapy Assistant program.
National Provider BLS - American Heart Association within 30 days of hire.
Oregon Occupational Therapist Assistant License upon hire.
Preferred: 1 year - Experience in the area of Occupational Therapy as a COTA.
