About the role
- Under the direction of the Supervisor/Manager triage incoming calls and assess patient needs
- Provide appropriate healthcare options and facilitate referral to primary providers, health care facilities and community resources
- Educate callers regarding immediate health care advice, possible health related risks, wellness and prevention behaviors
- Document and communicate care recommendations and referrals as required
Requirements
- Graduate of an accredited school of nursing
- Upon hire: California Registered Nurse License
- Case management or acute care experience in a healthcare setting
- 2 years of Acute or ambulatory nursing experience
- Preferred: Bachelor's Degree Nursing or related field
- Preferred: 2 years of Case management or acute care experience in a healthcare setting
- Preferred: 7 years of Acute or ambulatory nursing experience
