About the role

  • Reviews and evaluates each case by reviewing the services provided by the therapists, including case conferences and documentation review, discusses and verifies impressions, instructs and guides therapists to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist therapists as appropriate.
  • Assists therapists in establishing immediate and long-term rehabilitation goals, in setting priorities, and in developing plan of care.
  • Assists therapists in overseeing assignment and supervision of therapy assistants.
  • Attends case conference meetings with organization personnel to facilitate coordination of care.
  • Conducts quarterly record reviews and communicates findings and recommendations to Director of Nursing and appropriate organization personnel.
  • Completes on-site supervisory visits as necessary.
  • Maintain accountability for therapy staff productivity, utilization and documentation standards.
  • Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.
  • Assists in the orientation of new therapy personnel.
  • Assists Director of Nursing in the planning, implementation and evaluation of in-service and continuing education programs.
  • Assists in the formulation, revision, implementation, and evaluation of policies and procedures, as well as strategic goals and objectives.
  • Complies with accepted professional standards and principles.
  • Follow written and oral instructions.
  • Maintain patience, tact, cheerful disposition and enthusiasm.
  • Must comply with all training requirements.

Requirements

  • Licensed to practice as physical therapist within the State, if applicable.
  • Passed an examination for physical therapists approved by the state and possesses a degree from a baccalaureate or master’s program in physical therapy approved by the Commission on accreditation in Physical Therapy Education (“CAPTE”) or any successor organization of the CAPTE.
  • Two years management or supervisory experience in a rehabilitation therapy department or program, preferably home care.
  • Complies with accepted professional standards and practice.
  • Complies and maintains current CPR certification.
  • The ability to drive and/or have access to transportation for agency related travel (e.g. community partners, patients, etc.) is an essential function of the job. Must have and maintain a valid driver’s license, maintain automobile insurance coverage and have access to an automobile.
  • Demonstrates excellent observation, verbal and written communication skills.
  • Current state license as a Physical or Occupational therapist or Speech/Language Pathologist.
