Seeking a Lead Care Manager to outreach and engage individuals living with Serious Mental Illness/Substance Use Disorder, homelessness, and/or high medical needs.
This full-time role primarily remote; 1-2 on-site visits per month near your city.
Responsibilities include maintaining an ongoing caseload, using relationship-based strategies, telephonic and SMS outreach, developing Health Action Plan/Shared Care Plan, and coordinating care.
Collaborates with Enhanced Care Management team and meets monthly encounter metrics.
Requirements include 1+ years of general work experience (Case Management preferred), bilingual English/Spanish, California residency, and strong CRM/tech skills.
Preferred qualifications include lived experience with homelessness or SMI/ SUD, 2+ years case management, motivational interviewing, medical terminology knowledge.

Requirements
1+ years of general work experience (Case Management preferred)

Virtual Ops requires a quiet, HIPAA compliant and internet connected space
