About the role
- Performs patient care by assessing the patient's physical and functional abilities
- Establishes and carries out a plan of care which includes patient/family instruction, discharge planning, documentation on the medical record, and communication with the physicians and other medical staff
- Helps patients to achieve their maximum level of physical functioning
- Direct patient care and evaluates and treats patients after provider referral
- Plans and helps coordinate discharge plan with multidisciplinary team
- Documents evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team
- Takes active roles in orienting new staff members
Requirements
- Bachelor's Degree in Physical Therapy (Required)
- Driver's License if in a Home Care department
- Licensed in Field - State of Ohio
- Physical Therapist License - American Board of Physical Therapy Specialties
- Entry level new graduate, if facility has at least one veteran Physical Therapist to mentor, otherwise, at least one year of clinical Experience
