💰 $40 - $170 per hour

About the role

  • Provide in-home physical therapy evaluations and treatments
  • Assess patients’ strength, mobility, balance, and ability to function safely at home
  • Create and update individualized plans of care that support independence and recovery
  • Help patients regain mobility and confidence in their home environment
  • Educate patients and caregivers on exercises, safety, and fall prevention
  • Document visits and evaluations in WellSky/Kinnser in a timely manner
  • Collaborate with nurses, therapists, and the broader care team to support patient outcomes
  • Supervise Physical Therapy Assistants (PTAs) and support their clinical development
  • Participate in care planning, case conferences, and discharge planning

Requirements

  • Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or DPT)
  • Active California Physical Therapist license
  • CPR/BLS certification
  • Strong communication and organizational skills
  • Reliable transportation and willingness to travel to patients’ homes
  • Comfortable working independently in a home health setting
  • Home health experience preferred, but not required
