Medical Director

• Regulatory Oversight: Act as the registered Physician lead in accordance with California Title 22
• Clinical Quality Management & QAPI:
• - Actively participate in the Quality Management program
• - Attend Quarterly QAPI meetings to review clinical outcomes and patient safety data
• Clinical Governance and Consultation:
• - Conduct occasional chart reviews to ensure documentation meets medical necessity
• - Review and approve clinical protocols and treatment guidelines
• - Provide remote guidance to the Director of Patient Care Services (DPCS) on complex cases
• Laboratory Oversight: Ensure agency compliance with state CLIA California requirements

Medical Director, Home Healthcare – Part-Time

Therapist

• Provide in-home physical therapy visits under the supervision of a PT
• Help patients improve mobility, strength, balance, and functional independence
• Educate patients and caregivers on exercises and safe movement
• Document visits in WellSky/Kinnser in a timely manner
• Communicate patient progress and concerns to the care team

Physical Therapy Assistant – Home Health

• Provide in-home physical therapy evaluations and treatments
• Assess patients’ strength, mobility, balance, and ability to function safely at home
• Create and update individualized plans of care that support independence and recovery
• Help patients regain mobility and confidence in their home environment
• Educate patients and caregivers on exercises, safety, and fall prevention
• Document visits and evaluations in WellSky/Kinnser in a timely manner
• Collaborate with nurses, therapists, and the broader care team to support patient outcomes
• Supervise Physical Therapy Assistants (PTAs) and support their clinical development
• Participate in care planning, case conferences, and discharge planning

Physical Therapist – Home Health

• Provide in-home occupational therapy visits under the supervision of an Occupational Therapist
• Help patients improve mobility, strength, balance, and functional independence
• Educate patients and caregivers on exercises and safe movement
• Document visits in WellSky/Kinnser in a timely manner
• Communicate patient progress and concerns to the care team
