Program Manager

• Oversee all milieu activities
• Provide leadership, supervision and evaluation of program staff
• Assist the Clinician in creation of psychosocial assessments and treatment plans for the youth
• Connect with the parents of the enrolled children to provide updates on their child’s progress
• Manage risk issues of program staff as well as the children
• Evaluate the training needs of the program
• Represent the program at appropriate Department of Mental Health meetings and any school based or care coordination meetings for the enrolled child

Program Manager – After School Program

Customer Support

• Offer guidance for residents during skills building
• Develop and implement individual treatment plans for residents
• Assist with daily living activities, including meal preparation, money management, doctor’s appointments and assisting with personal hygiene
• Engage people and partner with them to improve their health and wellness, achieve symptom stabilization and self-management and develop and/or restore skills impacted by mental illness to help them live a self-directed life.
• Ensure appropriate staff-to-client ratio at all time
• Develop and promote a supportive relationship with the residents
• Provide side-by-side support and coaching to help people socialize and become more involved in their community.

Residential Support Specialist

• Responsible for the clinical and administrative aspects of a residential program for adults living with developmental disabilities
• Help people in the areas of daily living skills, transportation, skill-building, money management, medication administration, risk mitigation and other areas based on the person centered treatment plan
• Recruit, train, supervise, evaluate, and facilitate progressive discipline as needed for all staff in conjunction with the Residential Services Coordinator
• Arrange staff schedules to meet the health and safety requirements, as well as the individual needs of the people
• Create a Positive Behavior Supports atmosphere in the programs, following the PBS model
• Oversee use of the ISP system to complete ISP planning and ensure related documentation is completed in a timely manner
• Participate in the assessment of person referrals, admissions and discharges
• Ensure the program and person records are current, comprehensive and meet agency and funding source standards
• Liaison with families/guardians, DDS, and other outside service providers
• Supervise and participate in the housekeeping and maintenance of the house and vehicle
• Participate in the development of the program budget, and monitor the program expenses
• Monitor medications in accordance with MAP regulations
• Teach NSCS trainings to newly hired employees

Residential Program Manager

Counselor

• Be awake and available to persons during overnight shift until persons have left residential program, or until the morning staff person arrives
• Check in with staff, persons who are awake and complete a walk-through of the program every hour
• Read the Staff Communication Log and review risk plans
• Document each person’s progress in the confidential record
• Perform household maintenance and upkeep as needed
• Write daily notes at end of shift if needed, noting any incidents and maintain regular communication with other team members
• Complete medication counts

Residential Awake Overnight Counselor

Therapist

• Provide clinical assessments, evaluation, diagnosis and treatment of children using individual and group techniques
• Participate with/or supervises staff for assessments and interventions
• Administer and interpret client assessment activities as needed
• Function fully as part of an inter-disciplinary team and attend staff meetings/case conferences
• Participate in service coordination activities as a team member
• Prepare written reports as needed
• Maintain all required DPH, agency and program required documentation and ensure all client file placement
• Share knowledge and resources about involved pediatric occupational therapy intervention to client’s parents and staff
• Provide consultation, training, and in-services to staff as needed

Pediatric Occupational Therapist

Affiliate Manager

• Supervise children during program activities
• Intervene during a crisis using appropriate techniques including WELLE restraint.
• Drive program van and accompany other staff during ride.
• Complete daily documentation of children’s progress in the milieu.
• Perform duties as a Family Partner including maintaining contact with caregivers and conduct outreach to their families in their homes and communities as necessary.
• Other duties as assigned by program manager.

Family Partner

• Manage employee efforts to engage people and partner with them to improve their health and wellness
• Ensure that day to day operations of the group home program are grounded in the philosophy and techniques of psychiatric rehabilitation
• Assist people with psychiatric and substance use disorders to identify personal priorities, preferences, strengths, and interests in order to help them establish goals that support a life in recovery
• Instill hope by engaging in positive interactions that promote recovery and empowerment
• Collaborate in the development and implementation of Safety Plans and Crisis Prevention and Response Plans
• Lead the hiring process for residential staff and ensures that all staff are properly oriented and are trained in all mandatory trainings
• Participate in the on-call rotation

Residential Program Director

Bilingual

• Provide support, coordination of disposition planning and treatment service for the duration of the MCI episode of care according to clinical need
• Fulfill aspects of YMCI (Youth Mobile Crisis Intervention) ESP specifications, which require in-home crisis intervention with youth and families
• Coordinate follow up with other staff members of the Emergency Services Program to insure needs of youth and family are met to reduce crisis
• Provide direct mobile and on site psychiatric clinical services in coordination with other community service providers
• Evaluate clients in terms of setting and treatment plan which includes evaluation and developing a treatment plan
• Identify needs and strengths; assist in planning, implementing and evaluating necessary care of individuals, children and families in conjunction with community resources
• Asses crisis or emergency situations and intervene immediately with appropriate treatment for clients
• Prepare written/computer cases recording documents, within prescribed time limits

Mobile Crisis Clinician – Bilingual Spanish/Portuguese

• Perform security checks on an hourly basis to ensure safety of individuals and facility
• Document each person’s progress in the confidential record
• Perform tasks on the cleaning checklist and all duties based on individual’s needs and schedule
• Write daily notes at end of shift if needed, noting any incidents and maintain regular communication with other team members
• Complete medication counts

Residential Overnight Support Specialist

• Work directly with individuals and staff to ensure that the needs of the Individuals living in the group home are met in all areas of service
• Supervise the house basic systems such as ISP and clinical plan implementation
• Ensure community integration, medication administration, laundry, shopping, and menu/meal planning
• Ensure the maintenance and safety of the group home
• With the Program Manager, recruit, hire, train, assign, direct, counsel, discipline and evaluate performance of the Group Home DSP Staff
• Maintain positive and regular communication with Individual’s family members

Residential Assistant Program Manager

Manager

• Supervise and/or participate with residents, based on their needs in self-help skills, activities of daily living, socialization, leisure time and community integration.
• Assist the individual’s team to develop an IAP (Individualized Action Plan).
• Implement, teach, and assist individuals with IAP goals and objectives on a consistent basis.
• Document and maintain all individual residents’ records.
• Ensure proper administration and documentation of all medication.
• Interface and liaison with external providers and resources, i.e. Courts, DCF, Recovery Coaches, outpatient therapist and prescribers etc.
• Assist and approve weekly activity logs for each resident assigned.
• Work with treatment team to ensure cohesive daily communication.
• Assist residents with benefits.
• Assist and Support assigned residents on budgeting.
• Support residents with compliance of the medication procedure policy.
• Participate in upkeep and maintenance of program.

Case Manager – Residential Recovery Program
