About the role
- Provide physical therapy to patients in their homes
- Collaborate with healthcare professionals to coordinate patient care
- Assess patients' conditions and develop treatment plans
- Maintain accurate patient records and documentation

Requirements
- Education: Associate Degree or equivalent in Physical Therapy from an approved curriculum
- Experience: Two to four years' experience preferred
