About the role

  • Assesss, plans and implements care strategies individualized by member and directed toward the most appropriate and least restrictive level of care
  • Collaborate with member, family and healthcare providers to develop an individualized plan of care
  • Conduct individual counseling and group therapy with adolescents and adults
  • Identify and initiate referrals for social service programs – including financial, psycho-social, community and state supportive services
  • Advocate for members and families to ensure needs and choices are represented and supported by the healthcare team
  • Utilize approved clinical criteria to assess and determine appropriate level of care
  • Document all member assessments, care plans and referrals provided
  • Responsible for achieving set goals; Key Performance Indicators (KPIs)
  • Learn the StrongWell model and take responsibility and ownership for outcome based care
  • Participate in interdisciplinary team meetings and utilization management rounds
  • Coordinate care across a range of substance use disorder treatment settings
  • Promote responsible and ethical stewardship of company resources
  • Maintain excellent punctuality and attendance during work hours
  • Provide services remotely with a flexible schedule and autonomy to create groups and use evidence-based practices

Requirements

  • LCSW, LMFT, LMHC, in Washington State
  • Applicant must be licensed in Washington State (can live anywhere in the US)
  • Preferred experience with substance abuse population
  • Background in maternal; substance abuse preferred
  • Experience with adults and adolescents
  • Familiarity with Medicare and Medicaid procedures
  • Remote Work Experience
  • Ability to learn and apply the StrongWell model
  • Experience providing individual and group therapy
