• Assesses, plans and implements care strategies that are individualized by member and directed toward the most appropriate and least restrictive level of care
• Collaborates with member, family and healthcare providers to develop an individualized plan of care

• Efficiently accept patient handoffs from an Intake Care Manager and seamlessly continue care
• Develop rapport with patients to create a safe therapeutic environment
• Educate patients on medication, and the importance of medication adherence
• Match daily calendar to documentation in Mindoula's electronic medical record - MMR
• Monitor, evaluate, and record the patient’s progress using validated assessments each month
