About the role

  • Remotely serve substance-exposed members in Washington State within the Substance Use Disorder Program
  • Provide biopsychosocial assessments, individual counseling and group therapy for adolescents and adults
  • Assess, plan and implement individualized care strategies directed toward appropriate and least restrictive level of care
  • Collaborate with member, family and healthcare providers to develop individualized plans of care and coordinate across treatment settings
  • Identify and initiate referrals for social service programs (financial, psycho-social, community and state supportive services)
  • Advocate for members and families to ensure needs and choices are represented and supported
  • Utilize approved clinical criteria to assess and determine appropriate level of care
  • Document all member assessments, care plans and referrals provided
  • Responsible for achieving set goals and Key Performance Indicators (KPIs)
  • Learn the StrongWell model and take ownership for outcome-based care
  • Participate in interdisciplinary team meetings and utilization management rounds to assist with safe transitions of care
  • Promote responsible and ethical stewardship of company resources
  • Maintain excellent punctuality and attendance during work hours

Requirements

  • LCSW, LMFT, or LMHC license in Washington State
  • Must be licensed in Washington State
  • Can live anywhere in the United States
  • Preferred experience with substance abuse population
  • Background in maternal and substance abuse preferred
  • Experience with adults and adolescents
  • Familiarity with Medicare and Medicaid procedures
  • Remote work experience
  • Ability to conduct individual and group therapy
  • Ability to perform biopsychosocial assessments
  • Ability to coordinate care across substance use disorder treatment settings
