About the role
- Manage inbound referrals from health system and provider partners, ensuring accurate intake and documentation
- Engage referred patients and assist with scheduling their Midi appointments
- Ensure referrals move through established workflows efficiently and without delays
- Serve as a point of contact for patients navigating care between Midi and its health system and provider partners
- Answer patient questions about Midi services, referral pathways, and next steps in care
- Provide clear, supportive communication to ensure a positive patient experience
- Manage the dedicated Zendesk inbox and voicemail for inbound referral communications
- Share updates and operational feedback with the Commercial team regarding referral activity and workflow improvements
Requirements
- 2–4 years of experience in care coordination, referral management, patient navigation, or patient support
- Experience working in a medical practice, healthcare organization, or digital health environment
- Daily hands-on experience using Zendesk or a similar ticketing platform to manage patient or customer support queues
- Strong experience communicating with patients via phone and written channels
- Comfortable managing high volumes of inbound requests and patient communications while maintaining a positive patient experience
- Ability to follow structured workflows and SOPs while identifying opportunities to improve operational processes
- Experience working with EHR systems and operational tools
- Strong communication and cross-functional collaboration skills
- Comfort working in a fast-paced, growing healthcare organization.
