About the role
- Provide a member-centric, evidence-based model of care across multiple products
- Conduct comprehensive assessments
- Develop, implement, and monitor individualized care plans
- Coordinate services across the healthcare continuum
Requirements
- Bachelor's degree in Nursing or equivalent work experience in related field
- 5 years of work experience beyond degree of clinical/acute care nursing
- Current, unrestricted RN license in the state of residence
- Certified Case Manager (CCM) preferred, or ability to obtain within two years of hire
- Experience working with vulnerable and complex populations
