About the role
- Assist patients with education and assessment of their ongoing care management
- Enhance quality of patient management and maximize patient satisfaction
- Promote cost-effectiveness and address healthcare disparities
- Engage and care for medically complex patients with empathy
- Update team with important patient issues that affect overall health
- Stay up to date with knowledge in this space
- Work remotely while being available for in-person interactions as needed
Requirements
- Associates Degree (required)
- Bachelor's Degree (preferred)
- 1+ Years in outpatient Behavioral Health field and/or Social Support field
- 2+ Years in direct care management position conducting biopsychosocial assessments and care plan development (preferred)
- Understanding of how to navigate Federal and State programs (disability, food stamps, IHSS)
- Comfortable managing more than 80 patients
- Bilingual (preferred)
