About the role
- Full-time Patient Care Manager based in California, working remotely with in-person housing support as needed
- Assist patients with housing stability, social services navigation, and ongoing care management
- Focus on enhancing patient well-being, improving access to resources, and addressing housing-related healthcare disparities
- Specialize in medically complex patients and integrate housing support into care plans
- Responsibilities include securing and maintaining stable housing, connecting patients with HUD/Section 8/emergency rental assistance, collaborating with healthcare teams, educating patients on financial resources, and providing regular updates to care team
Requirements
- Associate’s Degree (required)
- Bachelor’s Degree (preferred)
- 2+ years of experience in housing support services (required)
- Strong knowledge of federal and state programs, including disability benefits, food assistance, and IHSS
- Bilingual candidates encouraged to apply
