About the role
- Coordinate and participate in home visits and care conferences
- Collaborate with RNCM to coordinate acute and primary care services
- Conduct in-person comprehensive assessments of members
- Develop and monitor member-centered plans
- Implement risk mitigation strategies to promote health and independence
- Maintain member records as required by DHS contract
Requirements
- Certified Social Worker in the State of Wisconsin
- Minimum of one (1) year experience with family care target populations
- Four-year bachelor’s degree or more in Human Services or related field
- Working knowledge of computers, programs, typing, and data entry
- Ability to access members’ homes not compliant with ADA regulations
- Ability to multi-task in a fast-paced environment
- Current driver’s license and insurance
- Health insurance
