About the role
- Advocate for the patient and family throughout the entire episode of care.
- Monitor patient’s progression intervening as needed to ensure efficient patient progress.
- Coordinate all aspects of discharge planning and establish a safe discharge plan.
Requirements
- BSN
- 2 years of medical surgical nursing experience
- 1-2 years of acute care, homecare or population health RN case management experience (preferred)
- CCM certified (preferred)
