About the role
- Uses the nursing process to address the physical, psychological, emotional, and social needs of patients
- Collaborates, communicates and coordinates care for patients and populations across settings
- Develops and implements a plan of care based on individual context
- Partners with patients and their families in identifying learning needs and providing education
- Minimizes risk of harm to patients by following established nursing practices and protocols
- Manages patient care coordination and care transitions to prevent readmissions
Requirements
- Current licensure as a Registered Nurse in state of employment required
- Bachelors Nursing preferred
- 7-10 years of nursing experience
- CCM, CMC, or ACM preferred
- Strong command of computer programs including Word, Excel, and PowerPoint
- Website development software and Photoshop preferred
- Strong written & verbal communications and organizational skills
