About the role

  • Assist with resident assessments and monitor changes in condition; report findings to the DON as appropriate
  • Support implementation and ongoing review of service plans and care needs
  • Maintain accurate and timely resident records and clinical documentation
  • Communicate changes in resident status to physicians, families, and leadership
  • Assist with coordination of resident services with internal and external care providers
  • Support safe and accurate medication administration and documentation in accordance with regulations and facility policy
  • Participate in review of medication records for accuracy and compliance
  • Monitor vital signs, weights, and other health indicators as assigned
  • Provide guidance and support to Resident Assistants regarding resident care procedures
  • Participate in new employee clinical orientation and ongoing training as directed
  • Model and promote a positive, team-oriented work environment
  • Participate in on-call or emergency response support as assigned
  • Assist in maintaining compliance with state, federal, and licensing regulations
  • Attend meetings and trainings as scheduled and support continuous quality improvement

Requirements

  • Current and active LPN license in the State of Wisconsin (required)
  • Prior experience in assisted living, memory care, home health, or long-term care preferred
  • Strong organizational, problem-solving, and clinical judgment skills
  • Compassionate, resident-focused approach with a passion for serving seniors
  • Excellent verbal and written communication skills
  • Ability to work independently and as part of a collaborative leadership team
