About the role
- Assist with resident assessments and monitor changes in condition; report findings to the DON as appropriate
- Support implementation and ongoing review of service plans and care needs
- Maintain accurate and timely resident records and clinical documentation
- Communicate changes in resident status to physicians, families, and leadership
- Assist with coordination of resident services with internal and external care providers
- Support safe and accurate medication administration and documentation in accordance with regulations and facility policy
- Participate in review of medication records for accuracy and compliance
- Monitor vital signs, weights, and other health indicators as assigned
- Provide guidance and support to Resident Assistants regarding resident care procedures
- Participate in new employee clinical orientation and ongoing training as directed
- Model and promote a positive, team-oriented work environment
- Participate in on-call or emergency response support as assigned
- Assist in maintaining compliance with state, federal, and licensing regulations
- Attend meetings and trainings as scheduled and support continuous quality improvement
Requirements
- Current and active LPN license in the State of Wisconsin (required)
- Prior experience in assisted living, memory care, home health, or long-term care preferred
- Strong organizational, problem-solving, and clinical judgment skills
- Compassionate, resident-focused approach with a passion for serving seniors
- Excellent verbal and written communication skills
- Ability to work independently and as part of a collaborative leadership team
