• Conduct structured phone-based interactions with patients on behalf of partner healthcare organizations
• Using software, follow evidence-informed protocols to check in on patients, complete assessments, and escalate concerns when needed
• Complete a minimum of 6 hours of patient outreach per week (approximately 12–18 calls), on a schedule you build around your coursework
• Conduct structured telephonic assessments and wellness check-ins using The Grove (Grapefruit Health’s platform)
• Document interaction outcomes accurately within the platform
• Escalate patient safety concerns to a licensed clinical supervisor in real time
• Maintain strict HIPAA compliance and patient confidentiality at all times
• Participate in onboarding training and ongoing check-ins
