About the role

  • - Assist individuals in identifying safe, suitable, and affordable housing opportunities
  • - Support clients with applications, documentation, and housing eligibility verification
  • - Coordinate apartment viewings, lease signings, and move-in logistics
  • - Build and maintain relationships with landlords, property managers, and housing providers
  • - Provide ongoing tenancy support to ensure long-term housing stability
  • - Educate clients on lease obligations, budgeting, and tenant responsibilities
  • - Address tenancy challenges and intervene early to prevent housing disruption
  • - Support clients in developing independent living skills
  • - Maintain a caseload of approximately 30-45 clients
  • - Conduct home visits and community-based meetings
  • - Develop individualized housing stability plans
  • - Coordinate with care teams and community partners to provide holistic support
  • - Maintain timely and accurate documentation of services and client interactions
  • - Track housing placements, outcomes, and program metrics
  • - Participate in team meetings, case consultations, and professional development

Requirements

  • - Bachelor’s degree in social work, human services, public health, or related field preferred
  • - Minimum 2 years of experience in case management, housing navigation, or social services
  • - Experience working with vulnerable populations preferred
  • - Knowledge of affordable and supportive housing systems is a plus
  • - Strong organizational, communication, and problem-solving skills
  • - Ability to manage a 30-45 client caseload in a field-based environment
  • - Proficiency with Microsoft Office and electronic documentation systems
  • - Reliable transportation and ability to travel throughout the Bronx for client visits and housing meetings
  • - Must have access to personal computer and high-speed internet
