About the role

  • Perform home quality visits in Adult Foster Care members’ homes to assess care and needs
  • Conduct monthly visits for members On Care with FreedomCare to address changes and provide support and training to caregivers
  • Provide support to caregivers and review services according to scope of Adult Foster Care and plan of care
  • Respect client and employee rights, ensure privacy and security of protected information and comply with agency policies
  • Follow FreedomCare clinical policy and procedures
  • Assess and reassess client needs and responses to care; perform annual home inspections for qualified setting safety and fire & safety documentation
  • Provide education and training to caregivers in-person and via LMS
  • Review AFC caregiver logs (daily notes) at minimum every 30 days
  • Develop, implement, evaluate, revise, and provide input to client plans of care and physician treatment plans
  • Complete all client documentation accurately, timely, and legibly after visits
  • Coordinate client care with physicians, RNs, Program Director and other disciplines
  • Interpret agency philosophy and objectives to clients, families, and others
  • Perform other job-related duties as required or directed by supervisor

Requirements

  • Bachelor’s degree and a social worker license from the Massachusetts Board of Registration in Social Work and at least two years of recent experience working with elders or adults with disabilities
  • OR bachelor’s degree and two years of clinical experience in the care of elders or people with disabilities
  • Bilingual in Spanish preferred
  • Experience with CRM (Salesforce preferred)
  • Valid driver’s license with an insured vehicle for travel within the state
  • Strong communication and interpersonal skills
  • Problem-solving and decision-making abilities
  • Organizational and multitasking skills
  • Excellent communication, critical thinking, and organizational skills
  • Experience using a CRM (Salesforce is a plus)
  • Self-motivated and able to thrive in a fast-paced start-up environment
  • Staff is not related to or legally responsible for a member receiving AFC
