About the role
- Evaluates patients referred to physical therapy, as well as the Childhood Obesity clinic, and Early Intervention
- Communicates with patient and/or parents/caregivers regarding evaluation results, treatment plan, and goals
- Provides home program activities to patients and/or parents/caregivers
- Engages patients and/or parents/caregivers in the treatment sessions
- Provides treatment to assigned patients
- Monitors progress of treatment goals through periodic observations and assessments
- Consults with Physician, allied health professionals, and other team members
- Participates in case conferencing meeting and in program planning meetings
- Communicates regularly with Referral Specialist, Case Management, Care Coordination, and Scheduling staff
- Completes all required documentation accurately, in a timely manner, and thoroughly
Requirements
- California State licensure as a Physical Therapist
- Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training
- At least 2 years’ experience as a Physical Therapist preferred
- Pediatric specific experience required and experience in early intervention (0-3) preferred
- Valid California driver’s license, an automobile, and proof of minimum levels of car insurance as required under California law
- Acceptable driving record
