About the role
- Ennoble Care is a mobile primary care, palliative care, and hospice service provider delivering home-based care
- Full-time Social Worker Care Manager supporting mental and physical health care for assigned patient caseload in Griffin, GA region
- Screen and assess patients for mental health and substance abuse, provide education, and facilitate follow-up care and referrals
- Track treatment response, support psychotropic medication management, provide brief evidence-based behavioral interventions
- Participate in caseload consultation with psychiatric consultant and document in EHR/registry
- Facilitate treatment plan changes, relapse prevention plans, and referrals to social services
- Comfortable with in-person visits and telephonic care; use Excel, Word, and EMR for assessments and productivity tracking
Requirements
- The Social Worker Care Manager will have a minimum of MSW
- Preferred Licensure or provisionally licensed
- Behavioral Health experience and knowledge
- Must be licensed driver with an insured automobile in good working order
- Ability to communicate effectively, both verbally and in writing.
- Time Management skills
