Run SUD TOC Groups (Substance Use Disorder Transition of Care) three times a week at Worcester Area detoxes
Support Members discharging from detox accessing treatment
Complete Utilization Management for inpatient and outpatient level of care
Perform case management telephonically and/or by home visits within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs
Respond to more complex cases and account specific requests
Use appropriate screening criteria knowledge and clinical judgment to assess member needs
Conduct assessments to identify individual needs and develop specific care plan to address objectives and goals
Monitor and evaluate effectiveness of care plan and modify plan as needed
Support member access to appropriate quality and cost-effective care
Coordinate with internal and external resources to meet identified needs of the members and collaborate with providers
Serve as a resource to other BH Case Managers
Participate in cross-functional teams, projects and initiatives
Required to go to Worcester, MA and surrounding areas detox centers to run group meetings 3 or more days a week
Requirements
Requires MA/MS in social work counseling or a related behavioral health field or a degree in nursing
Minimum of 3 years of clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment
Or any combination of education and experience which would provide an equivalent background
Current active unrestricted license such as RN, LCSW, LMHC, LICSW, LPC (as allowed by applicable state laws), LMFT, LMSW (as allowed by applicable state laws), or Clinical Psychologist required to practice within scope of licensure
Managed care experience required
Previous experience in case management and telephonic and/or in person coaching preferred
Substance abuse and detox experience desired
Experience in health coaching and motivational interviewing techniques preferred
For Government business only: LAPC, LAMFT (as allowed by applicable state laws) also acceptable
For associates in Puerto Rico who are member/patient facing: current PR health certificate and current PR Law 300 certificate required
Must be within a reasonable commuting distance from the posting location(s) unless accommodation is granted as required by law
