Design, implement and evaluate population health initiatives to increase health outcomes for designated sub-populations through data analysis, community collaboration and member engagement
Work with business partners to identify and implement small to medium projects that support the population health management strategy
Apply project management knowledge, skills, tools and techniques to project deliverables, processes, and systems within defined parameters using project management methodology
Lead and manage community, provider, and partner relationships to support population health initiatives and drive measurable change
Develop and maintain essential planning documents, MOUs, and contracts to ensure alignment and accountability
Secure project approvals and create detailed communication management plans
Support regulatory and quality reporting for population health teams
Define project team roles and responsibilities, develop structured work plans, and comprehensive schedules
Oversee deployment planning, facilitate regular project team meetings, and maintain/update all project-related documentation throughout the lifecycle
Identify, document, and prioritize scope changes; manage the approval process and resolve issues through a structured issue log
Ensure right skill sets aligned to project needs; set/manage expectations with resource managers and provide performance feedback
Execute communication plans and provide regular project updates to stakeholders
Track ROI, manage project budgets, monitor time against plan, develop and oversee testing strategies, and conduct project close-out and quality reviews
Perform additional responsibilities as assigned
Requirements
Requires a BA/BS in a related field
Minimum of 3 years professional project management experience, with at least 2 years leading and directing project tasks
Or any combination of education and experience which would provide an equivalent background
Demonstrated success in building strategic partnerships and collaborations that improve population health outcomes (preferred)
Demonstrated ability to create an effective team environment, build strong relationships, solve problems and issues in a matrix environment, and communicate effectively at all levels of an organization (preferred)
Project Management Professional (PMP) certification (preferred)
Experience working in a lead role on one or more projects (preferred)
Experience leading multiple projects simultaneously (preferred)
Excellent communication skills across all levels of an organization (preferred)
Must be able to work first shift 8:00 am-5:00 pm EST Monday thru Friday
Must be within reasonable commuting distance of an Ohio office; travel within the state of Ohio as needed
