Manage and oversee assigned care managers ensuring fidelity to the CFSP Care Management model
Manage resource utilization to ensure appropriate delivery of care and adequate coverage
Coordinate service delivery including member assessment of physical and psychological factors
Foster strong relationships with the Department of Social Services to support placement identification and transitions
Streamline placement transition processes and expedite for youth at risk of placement disruption
Participate in cross-functional workgroups to maintain and develop program
Evaluate Special Program support functions and recommend efficiency improvements
Develop and conduct training programs for staff involved in the program
Extract and manipulate analytical data to present findings to markets and stakeholders
Hire, train, coach, counsel, and evaluate performance of direct reports
Work virtually from home covering DSS Regions 4-7 with hours 11:00 am - 8:00 pm and some travel required
Requirements
North Carolina residency is required
Active and current license as an LCSW, LCMHC, LPA, LMFT, or RN issued by the state of North Carolina
MS/MA in social work, counseling, or a related behavioral health field, or a degree in nursing
Three (3) years of experience providing care management, case management, or care coordination to individuals served by the child welfare system
Unable to accommodate LCSW-A, LCMHC-A or any other associate level licenses
Preferred: Knowledge of resources, supports, services and opportunities for safe community living, including LTSS, Behavioral Health, therapeutic, and physical health services
Preferred: Experience working with Children, Youth, and Families served by Local Departments of Social Services (Foster Care and Adoptive Assistance)
Preferred: At least 2 years of management/supervisor experience with direct reports
Preferred: Service delivery coordination, discharge planning, or behavioral health experience in a managed care setting
