About the role

  • Deliver high-quality, in-home physical therapy services aligned with Elara Caring’s mission and standards
  • Evaluate patient functional needs, establish measurable goals, and adjust care plans to achieve optimal outcomes
  • Design and implement individualized plans of care with patient and caregiver involvement
  • Collaborate with interdisciplinary team members to support patient progress
  • Provide direction to support staff to help patients meet functional goals
  • Maintain accurate, timely documentation including evaluations, treatment plans, and progress notes
  • Serve as a clinical resource and advocate for physical therapy within the community

Requirements

  • Graduate of an APTA-approved Physical Therapy program
  • Current, unrestricted Physical Therapist license in the state of practice
  • Minimum one (1) year of experience in a healthcare setting
  • Ability and willingness to travel within the assigned coverage area (up to 50%)
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Ability to sit, stand, bend, lift, and move intermittently; must be able to lift 50–100 lbs
