About the role
- Deliver individualized therapy treatments in patients’ homes under the established Plan of Care
- Help patients improve mobility, balance, and strength to safely navigate daily life
- Educate patients and caregivers on exercises, safety techniques, and proper use of equipment
- Observe, document, and communicate patient progress and any changes to the supervising PT
- Follow all state practice guidelines and Elara Caring clinical standards
- Assist with transfers, mobility, and hands-on care as needed in a home-based environment
Requirements
- Graduate of an accredited Physical Therapist Assistant program
- Active Michigan PTA license in good standing
- Minimum 6 months of PTA experience (home health experience preferred)
- Reliable transportation, valid driver’s license, and current auto insurance
- Comfortable traveling within the assigned coverage area
- Ability to meet physical demands of the role, including lifting and assisting patients
