About the role

  • Provide individualized therapy services to patients in their homes
  • Complete in-home physical therapy evaluations and functional assessments
  • Develop and carry out personalized treatment plans based on patient needs and goals
  • Provide hands-on therapy to improve strength, mobility, balance, and safety
  • Monitor patient progress and adjust care plans as appropriate
  • Collaborate with physicians, nurses, and interdisciplinary care team members
  • Educate patients and caregivers on exercises, safe movement, and fall prevention
  • Maintain timely, accurate clinical documentation
  • Support continuity of care and positive patient outcomes

Requirements

  • Graduate of an APTA-accredited Physical Therapy program
  • Active, unrestricted Physical Therapist license in the state of practice
  • Minimum of one year of clinical experience (home health experience preferred)
  • Strong clinical judgment and comfort working independently
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Ability to travel within the assigned territory
  • Physical ability to assist, lift, and transfer patients (up to 50–100 lbs)
