About the role
- Provide individualized therapy services to patients in their homes
- Complete in-home physical therapy evaluations and functional assessments
- Develop and carry out personalized treatment plans based on patient needs and goals
- Provide hands-on therapy to improve strength, mobility, balance, and safety
- Monitor patient progress and adjust care plans as appropriate
- Collaborate with physicians, nurses, and interdisciplinary care team members
- Educate patients and caregivers on exercises, safe movement, and fall prevention
- Maintain timely, accurate clinical documentation
- Support continuity of care and positive patient outcomes
Requirements
- Graduate of an APTA-accredited Physical Therapy program
- Active, unrestricted Physical Therapist license in the state of practice
- Minimum of one year of clinical experience (home health experience preferred)
- Strong clinical judgment and comfort working independently
- Reliable transportation, valid driver’s license, and current auto insurance
- Ability to travel within the assigned territory
- Physical ability to assist, lift, and transfer patients (up to 50–100 lbs)
