About the role
- Conduct in-home evaluations to assess mobility, strength, balance, and functional limitations
- Create and adjust personalized therapy programs aligned with physician orders and patient goals
- Provide hands-on treatment focused on restoring independence and preventing re-injury
- Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated care
- Educate patients and caregivers on safe mobility, exercises, and fall prevention
- Supervise and support therapy assistants as needed
- Complete accurate and timely documentation in compliance with agency and regulatory standards
- Serve as a trusted therapy resource within the communities you support
Requirements
- Degree from an APTA-accredited Physical Therapy program
- Active Physical Therapist license in the state of Michigan
- Minimum of 1 year of clinical experience (home health preferred, not required)
- Comfort working independently in the field with strong clinical judgment
- Reliable transportation, valid driver’s license, and auto insurance
- Ability to travel within the service area (up to 50%)
- Physical ability to assist, lift, and transfer patients (up to 50–100 lbs)
