About the role
- Provide individualized, in-home therapy services that help patients regain mobility, independence, and confidence
- Conduct in-home physical therapy evaluations and functional assessments
- Develop and implement personalized treatment plans aligned with patient goals and physician orders
- Provide hands-on therapy focused on strength, balance, gait, and mobility
- Modify care plans based on patient progress and changing clinical needs
- Collaborate with physicians, nurses, and interdisciplinary team members
- Educate patients and caregivers on safe mobility, exercises, and fall prevention
- Complete timely, accurate clinical documentation in accordance with agency standards
Requirements
- Graduate of an APTA-accredited Physical Therapy program
- Active, unrestricted Physical Therapist license in the state of practice
- Minimum of one year of clinical experience (home health experience preferred)
- Strong clinical judgment and comfort working independently
- Reliable transportation, valid driver’s license, and current auto insurance
- Ability to travel within the assigned territory
- Physical ability to lift, transfer, and assist patients (up to 50–100 lbs)
