About the role

  • Provide individualized, in-home therapy services that help patients regain mobility, independence, and confidence
  • Conduct in-home physical therapy evaluations and functional assessments
  • Develop and implement personalized treatment plans aligned with patient goals and physician orders
  • Provide hands-on therapy focused on strength, balance, gait, and mobility
  • Modify care plans based on patient progress and changing clinical needs
  • Collaborate with physicians, nurses, and interdisciplinary team members
  • Educate patients and caregivers on safe mobility, exercises, and fall prevention
  • Complete timely, accurate clinical documentation in accordance with agency standards

Requirements

  • Graduate of an APTA-accredited Physical Therapy program
  • Active, unrestricted Physical Therapist license in the state of practice
  • Minimum of one year of clinical experience (home health experience preferred)
  • Strong clinical judgment and comfort working independently
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Ability to travel within the assigned territory
  • Physical ability to lift, transfer, and assist patients (up to 50–100 lbs)
