About the role
- Provide occupational therapy services to patients in their homes under the supervision of an Occupational Therapist
- Follow and implement individualized plans of care to support patient independence and safety
- Monitor patient progress and communicate changes in condition to the OT, physician, and care team
- Educate patients and caregivers on therapy goals and home exercise programs
- Maintain timely, accurate clinical documentation
- Collaborate closely with interdisciplinary team members to ensure high-quality care
- Help reduce avoidable hospitalizations and ER visits through proactive, patient-centered care
Requirements
- Graduate of an accredited occupational therapy assistant program
- Active, unrestricted COTA certification in the state of practice
- Minimum 1 year of clinical experience as a COTA
- Comfortable working independently in a field-based role
- Ability and willingness to travel within the assigned coverage area (approximately 50%)
- Reliable vehicle, valid driver’s license, and current auto insurance
- Ability to lift up to 50–100 lbs and perform physical tasks associated with patient care
