About the role
- Deliver high-quality physical therapy services to patients in their homes
- Evaluate patient needs, develop individualized plans of care, and monitor progress
- Collaborate with physicians, caregivers, and interdisciplinary team members
- Adjust treatment plans to ensure optimal functional outcomes
- Supervise and guide support staff to help patients achieve therapy goals
- Maintain accurate, timely documentation and treatment records
- Serve as a clinical resource and advocate for physical therapy within the community
Requirements
- Graduate of an APTA-approved Physical Therapy program
- Active, unrestricted Physical Therapist license in the state of practice
- Minimum 1 year of experience in a healthcare setting
- Comfortable working independently in a field-based role
- Ability and willingness to travel within the assigned coverage area (approximately 50%)
- Reliable vehicle, valid driver’s license, and current auto insurance
- Ability to lift up to 50–100 lbs and perform physical tasks associated with patient care
