About the role

  • Deliver high-quality physical therapy services to patients in their homes
  • Evaluate patient needs, develop individualized plans of care, and monitor progress
  • Collaborate with physicians, caregivers, and interdisciplinary team members
  • Adjust treatment plans to ensure optimal functional outcomes
  • Supervise and guide support staff to help patients achieve therapy goals
  • Maintain accurate, timely documentation and treatment records

Requirements

  • Graduate of an APTA-approved Physical Therapy program
  • Active, unrestricted Physical Therapist license in the state of practice
  • Minimum 1 year of experience in a healthcare setting
  • Comfortable working independently in a field-based role
  • Ability and willingness to travel within the assigned coverage area (approximately 50%)
  • Reliable vehicle, valid driver’s license, and current auto insurance
  • Ability to lift up to 50–100 lbs and perform physical tasks associated with patient care
