• Conduct nurse-led telehealth assessments and My Health Record (MHR) reviews to identify clinical risks and support safe patient care pathways.
• Manage a high-volume clinical workload, balancing patient communication, case reviews, and administrative priorities while maintaining quality and efficiency.
• Apply clinical reasoning to review clinical documentation and determine appropriate next steps within scope of practice.
• Maintain accurate clinical documentation and manage patient cases across multiple systems while ensuring compliance with privacy and regulatory standards.
• Coordinate with multidisciplinary teams, including practitioners, pharmacy, and operational teams, to facilitate safe and efficient patient care.
