About the role
- Collaborate with the multidisciplinary team to support clients within their homes.
- Assess the client’s current function/needs.
- Create and implement a tailored treatment plan to enable clients to regain independence and continue to live safely at home.
Requirements
- Are qualified Physiotherapists with a HCPC registration.
- Have the ability to organise, prioritise and delegate workload.
- Are able to respond positively to change and to implement change initiatives.
- Are motivated to work flexibly within an inter-professional framework.
- Have the ability to meet the travel requirements of the role.
