About the role
- Development and implementation of action plans to drive clinical results
- Monitor and conduct trend analysis of clinical outcomes
- Partnering with Home Leadership to implement action plans and monitor results
- Reviewing home remote monitoring data to assist patient management
- Working directly with home nurses to leverage home remote monitoring technology
- Report out on clinical outcomes and action plans to home leadership team
- Foster physician and nurse relationships to implement clinical initiatives
- Evaluating the training of Home Dialysis Nurses to identify opportunities
- Partnering with Home Dialysis Nurse preceptors to train and develop Home Nurses
- Attend team meetings, phone conferences, and training as needed
- Travel to and from facilities within the assigned Division as needed
- Know, understand, and follow teammate guidelines, employment policies, and department or company procedures
- Other duties as assigned.
Requirements
- RN license in state of employment required or compact licensure
- Valid driver’s license in state of employment and positive driving record sufficient to meet insurance requirements of company required
- CPR certification required
