About the role
- Telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members
- Evaluate medical needs of members to facilitate overall wellness
- Manage caseloads including supportive and medically complex members
- Develop proactive plans to enhance short- and long-term member outcomes
- Determine and coordinate services such as Prior Authorizations, PCP coordination, condition management, medication review, and community resources
- Travel 50-75% to perform face-to-face visits with members within Seminole County and surrounding areas
- Coordinate care with providers and community supports to facilitate member wellness and integration
Requirements
- Must reside in Seminole County FL
- Willing and able to travel 50-75% for face-to-face member visits
- 1+ year of experience in behavioral health or a long-term care setting
- Bachelor's degree required, preferably in social work or related field
- Ability to multitask, prioritize and adapt to a fast-paced changing environment
- Effective verbal and written communication skills
- Managed Care experience (preferred)
- Computer proficiency in Microsoft Word, Excel, and Outlook required
- Case management and discharge planning experience
- Schedule availability Monday-Friday, 8:00am-5:00pm
